How 3D Preoperative Planning and Patient-Specific Guides Supported a Heterotopic Ossification Resection near the hip

In this case, the surgical team faced the challenge of removing a heterotopic ossification located close to the acetabulum — a highly constrained anatomical area where precision and control are essential.
From Preoperative Planning to Intraoperative Guidance
To support the surgical act, a patient-specific 3D guide was designed based on detailed preoperative planning.This plan defined:
Two cutting planes with precise orientation and depth
The use of six K-wires to secure the guide and serve as mechanical stops for depth control
A stepwise resection strategy, starting with the removal of small bridges before completing the cuts with a saw blade under guided control
Once the pathological fragment was freed, it could be removed cleanly, allowing the femur to be separated from the acetabulum as intended.
The patient-specific guide transformed a challenging procedure into a sequence of controlled and reproducible steps, faithfully mirroring the preoperative plan.
Planning That Supports, Not Replaces
It is essential to highlight that such preparation does not replace the surgical act — it supports it.
By providing a clear roadmap, it helps reduce variability in the operating room while leaving the precision of execution in the hands of the surgical team.
